First oral surgery visit

Welcome to 7x7 Dental Implant & Oral Surgery Specialists! Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as your consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Here are a few things that we will ask you to provide when you come in for your first appointment:

  • Your health and/or dental insurance information
  • A list of current medical conditions
  • A list of current medications
  • Referral forms or X-rays that your dentist or physician have provided (if any)
  • A valid driver’s license or photo ID
  • A form of payment (credit card, checkbook, cash, etc.)

You can expect to receive attentive care and be welcomed by friendly faces when you walk into any of our oral surgery offices. To speed up the registration process, you can fill out the medical/dental health questionnaire, the patient information form, and the financial information form prior to your visit. This will save time in the office, and if you have not filled out these forms online, we ask you to arrive a few minutes early to allow for time to complete them.

IMPORTANT: If you are under the age of 18, a parent or guardian must be present with you during your consultation visit.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia. We will provide you with detailed pre- and post-operative instructions before your procedure, which you will be expected to follow closely and carefully.

Pre-operative instructions will help you prepare for treatment, while post-operative instructions will be more specific to your procedure and provide direction for your care after treatment. We deliver elite oral surgery services at reasonable costs to our patients. Please remember that payment is due at the time service is rendered unless arrangements have been made in advance with our administrative staff. Most insurance companies will respond within 4 to 6 weeks. Any remaining balance after your insurance has paid is your responsibility. You are fully responsible for all fees charged by this office regardless of your insurance coverage. We are happy to make arrangements for a payment plan, but this must be discussed and implemented prior to your procedure.
